Insurance Information

Below is a list of insurances we are in network with or in the process of paneling with. We can bill your insurance company directly if you have Out-of-Network benefits, or offer a Superbill for insurance reimbursement. 

As a courtesy, we will conduct a preliminary verification of your insurance coverage with your insurance company. This information is our best estimate of your insurance coverage. It is ultimately your responsibility to check your own insurance coverage. Clients are responsible for fees incurred through services rendered. This includes when insurance denies a claim. We encourage clients to stay informed of your current coverage by directly contacting your insurance company. Please inform us of any upcoming changes.

In network providers with Aetna at MindTree Holistic Counseling include: Kathy Link, Amy Hughes, Tamar Kaplan  and  Jessica Redmond

In network providers with Blue Cross Blue Shield at MindTree Holistic Counseling include: Kathy Link, Amy Hughes, Jessica Redmond,  Tamar Kaplan.

In network providers with First Choice Health at MindTree Holistic Counseling include: Kathy Link  and Amy Hughes.

In network providers with Kaiser Permanente at MindTree Holistic Counseling include:  Amy Hughes, Kathy Link, Tamar Kaplan, and Jessica Redmond.

In network providers with Providence at MindTree Holistic Counseling include: Kathy Link, Tamar Kaplan, Jessica Redmond and Amy Hughes. 

In network providers with Moda at MindTree Holistic Counseling include: Amy Hughes, Tamar Kaplan and Kathy Link. Clinicians expected to be paneled at a later date: Jessica Redmond 

All other clinicians: Intake Assessment 60 min - $250, Counseling session 50 min - $205

Cancellations at MindTree

A 48 business-hour notice is required for cancellations. If you miss an appointment or cancel with less than a 48 business-hour notice, this is time that could be spent with another client; therefore, it is necessary to charge a full fee. Business hours do not include weekends. Please be aware that most insurance companies will not reimburse your missed appointment fee.

Good Faith Estimate

You are entitled to receive a Good Faith Estimate that shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created.

The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.

If you are billed for more than this Good Faith Estimate, you have the right to dispute the bill.

You may contact the health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill or ask if there is financial assistance available.

You may also start a dispute resolution process with the U.S. Department of Health and Human Services (HHS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date on the original bill.

There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you will have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with the health care provider or facility, you will have to pay the higher amount.
